Job Description
Community Solutions Inc. is looking for a dedicated and passionate individual to join our Case Management Team. You will be working at the Drapelick Center in Bloomfield CT .
You will become a part of a team helping to guide our clients in their development and assist them to become a positive influence in the community during their reentry. Some general duties for this position will include conducting group and individual counseling sessions in addition to keeping case records. You will also be responsible for completing and submitting discharge plans for all of your clients.
Schedule: M/W 1pm-9pm, T/TH/F 9am-5pm
Community Solutions Inc. Since CSI’s founding in 1962, it has been a pioneer in the community-based service. We have been delivering supervision, treatment, and comprehensive individualized services to help clients interact more effectively in their communities and become productive citizens. Using this plan we built the first ever halfway house in New England which has now been replicated and grown to almost 50 programs in 9 states. For over 50 years, CSI has been building upon its successful foundations and has been expanding its services to serve thousands of adults, youth, and families each year.
ESSENTIAL FUNCTIONS include but are not limited to:
Submit weekly progress reports for the first (6) six weeks and bi-weekly progress reports thereafter. Complete and submit discharge summary within 7 calendar days of discharge.
Home confinement clients: Collect and review phone records on a monthly basis to ensure compliance with the SOW.
Record all sessions with clients and follow procedure to upload these recordings for quality assurance review.
Actively participate in quality assurance review session and apply new strategies and techniques presented.
Actively participate in PRT meetings by presenting new clients’ assessments and ISP, updates on clients nearing discharge, provide and elicit peer support for client issues.
Maintain client files and ensure compliance with Agency policy regarding confidential information. This confidentiality must be maintained even if you are no longer an employee of CSI
On a monthly basis collect and make copies of the clients’ bank records to ensure it balances with our financial records.
Perform other tasks as assigned by the supervisor
Qualifications
Bachelor’s degree is Required
Excellent oral, written and interpersonal skills.
Ability to engage and develop collaborations with community agencies.
Previous assessment experience preferred.
Experience with Microsoft Office.
