Profile

The Therapist provides comprehensive assessments, coordinati on, planning, and implementati on of apsychosocial treatment plan for pati ents in respecti ve department (**MEMBERS ONLY**SIGN UP NOW***. Addicti on Treatment Center -HATC, Community Health, etc.). This role promotes opti mal mental health by providing ongoing psychosocialsupport and services to pati ents and their families for adjustments to illness, grief, addicti on and otherpsychosocial stressors. Works with an interdisciplinary team to achieve treatment goals. Uti lizes high-riskcriteria as well as medical records, school records, clients, families, nurses, physicians and others to evaluatepati ent's biopsychosocial needs. Develops a treatment plan and a comprehensive terminati on oftreatment/discharge plan. Documents objecti ve and accurate data regarding assessments andinterventi ons. Maintains working knowledge of community-based resources, programs and social services.Educates pati ents and families on their health care opti ons and connects them with appropriate resources.
May develop and facilitate support groups, and workshops as needed. May provide input on treatmentschedules, pati ent assignments, and/or crisis interventi ons as requested by Clinical Manager. Performs otherduti es as assigned.
In additi on to the above, the Therapist-Licensed provides ongoing teaching andsupervision to MSW employees and interns and may sign off on State requirements for LCSW licensing.Uti lizes advanced clinical knowledge to assess and identi fy psychiatric and psychosocial issues facingpati ents. May conduct assessments related to legal ethical reporti ng requirements. Some departmentsprovide additi onal specialist services such as Reiki Therapy when the clinician is certi fi ed to do so.
SolMar Retreat : In additi on to the above provides consistent group and individual evidencedbased therapy through modaliti es with assigned clients. Complete detailed and ti melydocumentati on for: biopsychosocial assessment, weekly goals, treatment & discharge planning,progress notes, group and individual sessions. Ensures conti nuity of care by maintaining regularcontact with families, outside clinicians, and referring parti es. Parti cipates in case management,care coordinati on, aft ercare, uti lizati on reviews, and treatment team meeti ngs.

 

Edducation & Experience: 

Master's Degree required. Working knowledge of Microsoft Office Suite required. Four years' experience inSocial Work in an acute healthcare system.
Additional Education and Experience required vary by discipline as follows:

*Community Health:Master's Degree in Social Work (MSW) or Marriage and Family Therapist (MFT) required.Experience with clinical assessment, crisis intervention, and case management required. Bilingual ormultilingual preferred. ***MEMBERS ONLY**SIGN UP NOW***. Addiction Treatment Center (HATC):Masters' Degree in Social work (MSW) or Marriage and FamilyTherapy (MFT) required. Minimum two (2) years' experience in addiction/behavioral health environment in ahospital or treatment center setting required. Experience with clinical assessment, crisis intervention, and casemanagement required. *Maternal Health Clinic:Master's Degree in Marriage and Family Therapy (MFT) required. Experience withclinical assessments and crisis intervention required. Knowledge of Cognitive Behavioral Therapy (CBT)required. Effectively working with culturally diverse clients required. Experience in the field of Women's MentalHealth preferred. Bilingual or multilingual candidates preferred. *Oncology and Palliative Care:Master's degree in Social Work (MSW) required. Experience with clinicalassessment and crisis intervention required. Bilingual or multilingual preferred. *Home Health:: Must be a licensed driver with an automobile that is insured in accordance with state and/ororganizati on requirements and is in good working order. Experience in home health care preferred. *Hospice:Must be a licensed driver with an automobile that is insured in accordance with state and/ororganizati on requirements and is in good working order. Experience in hospice care preferred. Understandshospice philosophy, and issues of death/dying. *HATC:Masters' Degree in Social work (MSW or MFT) required. Minimum two years' experience inaddiction/behavioral health. *SolMar Retreat:Masters degree in Social Work (MSW) or Marriage and Family Therapy (MFT)required.Minimum two years experience in Addiction /Behavioral health. Knowledge of CARF and/or JACHOstandards and 12 step, Smart Recovery treatment models. Knowledge of Incidental Medical Services/MATpolicies and procedures and HIPPA
 

Licensure:: 

*Acute Care Rehab, Cardiac Rehab, Oncology and Palliative Care: *LCSW - Licensed Clinical Social Worker (LCSW), in good standing, in the State of California  *Community Health, Maternal Health Clinic, HATC, SolMar and Chemical Dependency Op. Services : LCSW  *Licensed Social Worker (LCSW), in good standing, in the State of California or *LMFT - Licensed Marriage Family Therapist (LMFT), in good standing, in the State of California
 

Certifications Required:
